Having specialist care needs shouldn’t prevent a person from leading fulfilling and active lives. Our supported living service provides support to people of all ages with physical or learning disabilities or mental health needs.
Our care workers support you to enjoy your personal interests and live an independent and active life. With our specialised supported living service, you can lead the life you want to live, in and out of your own home.
Our supported living support is tailored to your individual needs, from daily living tasks, such as housework and shopping, meal preparation and managing finances, through to personal care. We also support you in maintaining your active social life outside the home and will assist with meeting friends and family, pursuing a hobby, eating well and fitness.
Your care plan is individually designed in partnership with you to provide the support you want and need, where and when you want it. We can simply pop in a couple of times a week or can provide live in 24 hour care.
Our key worker system allows you to choose the key worker who will be the point of contact for you and your principal care/relatives. They’ll be available to answer any questions, support your daily care routine or just chat with you when you wish.
Initially your key worker will help you to note your preferred name, personal circumstances, personal history and social and medical circumstances. They will also assess your psychological and physical capabilities, as well as noting previous work and hobbies, likes and dislikes (when it comes to activities and food for example). Your personal programme of support and/or care can then be developed.
He/she will continue to support your ongoing needs, arranging Service User Plan meetings with you and your family or friends, and measuring the progress of your care programme.
